Abstract

The dentigerous cyst, also known as follicular cyst, originates from the accumulation of fluid between the reduced epithelium of the enamel organ and the dental crown, which results in the separation of the follicle that surrounds the crown of an unerupted tooth. They are usually radiolucent and unilocular. The objective of this work is to present a case of dentigerous cyst associated with an impacted third molar causing resorption of the first molar. A 28-year-old female patient came to the teaching clinic with the main complaint of pain in the retromolar region for about 6 months and tenderness in the lower left first molar. Collection of Platelet Rich Fibrin (PRF), extraction of the lower first and lower left third molars, curettage for biopsy and obtaining a result compatible with dentigerous cyst were performed. The prognosis and results obtained in this case were positive. Radiographic and histopathological exams were chosen to complement the diagnosis. Histopathological examination should be performed, as the dentigerous cyst is similar to other lesions such as keratocystic odontogenic tumor and unicystic ameloblastoma. Platelet Rich Fibrins are inserted forming a plug in the injured tissue and are responsible for triggering the next stages of tissue regeneration. The techniques used to treat dentigerous cysts are enucleation and marsupialization, in 85% of cases, enucleation and extraction of the associated tooth are chosen. Faced with these cases, the dental surgeon must be prepared to carry out the diagnosis, treatment or referral to a specialist, when necessary.
