Abstract

Objective: To identify anatomical changes in diagnostic tests and the feasibility of surgical interventions in patients with Dextrocardia. Data sources: This is a systematic review of the Virtual Health Library (VHL) and PubMed. Descriptors in health sciences (Decs) were used combined with the Boolean operator “ANDâ€, in Pubmed, the descriptors were in English “Dextrocardia situs inversus totalis†AND “diagnosisâ€, using filters “full text†and “last 5 yearsâ€, In the VHL, the descriptors were in Portuguese with the same filters. Data synthesis: The anatomical variations in the diagnostic exams are: location of the heart in the right hemithorax, in which the apex of the organ, upper and lower vena cava are directed to the right and on the electrocardiogram there is a deviation of the axis to the extreme right with the presence of complexes QRS in the precordial plane inverted. For the execution of any suitable surgical intervention, a detailed anatomical diagnosis and preoperative evaluation must be determined with a prepared team. Surgical procedures in patients with dextrocardia are: robotic intervention to remove a thrombus in the right atrial appendage in a pregnant woman, myocardial revascularization, in-stent insertion in total coronary occlusions, lung transplantation and mitral, tricuspid, aortic and aortic valve surgery. Conclusion: It is concluded that prior knowledge of the anatomical alteration is necessary, being easily determined by means of imaging exams and electrocardiogram to conduct any case of dextrocardia. The study demonstrated interventional viability in usual intra and extra-cardiac surgical procedures, showing success in most of them.
